I've seen this on a few occasions when asking people to revise an entry so even if the explanation is user error it is a common enough user error that it may be worth giving some thought to how help files, dialogue boxes etc. are set up to avoid users making the error.
I asked an observer to change one of their records (this one https://irecord.org.uk/record-details?occurrence_id=29726044) where their ID was incorrect (I prefer to give users the option to do it themselves than override their ID). He did so adding a comment "edited to Spectacle on advice of verifying team." However that comment also got added to a bunch of his other records that he entered (for instance this one https://irecord.org.uk/record-details?occurrence_id=29726042) and it being added to them also reset those records verification status. Fortunately only a few in this case although I've previously seen it reset much larger numbers.
Not sure why it happened but as I say it's not the first time I've seen this happen when people revise some of their records and add comment.
