I'm not any kind of admin here, but I think I can answer this one.
While it is certainly true that such duplication is best avoided. My feeling is that detection of duplicates is a relatively straightforward process these days and anyone handling/using the data should be checking for them.
In theory all BSBI Vice-county Recorders should have access to iRecord and should manage the flow of data from iRecord to the BSBI DDb. However it rather depends how tech savvy and/or busy your local recorder is.
I do know that the BSBI database is pretty good at flagging duplicates, so I don't tend to worry about it.
